What Actually Causes Smoking-Related Cancer
Tobacco smoke contains over 70 known carcinogens: chemicals that cause cancer by damaging DNA in cells. These include polycyclic aromatic hydrocarbons, nitrosamines, benzene, formaldehyde, arsenic and radioactive polonium-210. These compounds are produced by the combustion of tobacco, not by nicotine itself.
Nicotine is the addictive compound that keeps people smoking and therefore keeps them exposed to the carcinogens in smoke. In this indirect sense, nicotine is responsible for cancer by maintaining the addiction that exposes people to carcinogens. But nicotine alone, delivered without combustion products, is not classified as a carcinogen.

The NRT Argument: Decades of Evidence
Nicotine replacement therapies: patches, gum, lozenges, inhalators: have been prescribed to millions of people including cancer patients, post-surgery patients and pregnant women for decades. If nicotine itself caused cancer at the levels delivered by NRT, this would have generated an observable clinical signal by now. The absence of such a signal from decades of widespread NRT use is strong practical evidence that nicotine alone does not cause cancer at normal therapeutic levels.
